Cancel an insurance policy

Dear Sir or Madam, I am writing to cancel the [Type of insurance] policy I hold with [Name of the insurer], under policy number [Policy number]. Please end the cover with effect from [Date the cover should end]. If the policy requires a longer period of notice, please end it on the earliest date allowed after that and confirm that date to me in writing. I do not wish the policy to renew automatically for a further term. Please refund any premium relating to the period after the cover has ended, and send me a closing statement showing how that amount has been calculated. From the end date onwards I withdraw my authority for any further collection of premiums. I would also ask you to send me a statement of my claims history, so that I can produce it when arranging cover elsewhere. Please confirm this cancellation within fourteen days.
