Cancel a gym membership

Dear Sir or Madam, I am writing to cancel my membership of [Name of the gym], held under membership number [Membership number]. Please bring the membership to an end on [Date the membership should end]. If the terms of the agreement require a longer period of notice, please end it on the earliest date permitted after that and tell me what that date is. I do not agree to the membership rolling over into a further fixed term. From the end date onwards I withdraw my authority for any direct debit or recurring card payment in favour of [Name of the gym]. Please cancel the mandate at your end as well, and refund anything collected in respect of a period after the membership has ended. Please send me written confirmation within fourteen days stating the date the membership ends, the date of the final collection, and any balance still outstanding. I would also ask you to stop sending me marketing messages once the membership has ended. If I have heard nothing within that period, I will treat the membership as having ended on the date given above and will instruct my bank to refuse any further collection.
